Zoladex injection contains the active ingredient goserelin acetate, which is a synthetic analogue of gonadotropin-releasing hormone (GnRH). Gonadotropin-releasing hormone stimulates the pituitary gland to produce and release luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which in turn stimulate the production of estrogen and testosterone. However, continuous administration of GnRH agonists like goserelin causes downregulation of the GnRH receptor, leading to a reduction in the production of these hormones.
Uses:
Zoladex is used in the treatment of advanced prostate cancer, breast cancer, and endometriosis. In addition, it is used in in vitro fertilization (IVF) to control ovulation and improve the success rate of fertility treatments.
Dosage and Usage:
The dosage of Zoladex injection depends on the condition being treated. For prostate cancer, a monthly dose of 3.6mg is administered as a subcutaneous injection into the abdominal area or buttocks. In breast cancer, the recommended dosage is 3.6mg every four weeks. In endometriosis, the recommended dose is 3.6mg once every four weeks for three to six months. Patients must prepare the injection site with an alcohol swab and follow the instructions provided by the healthcare provider.
Storage conditions:
Zoladex injection should be stored in a refrigerator between 2°C and 8°C before use. If unavailable, the unopened vial can be stored at room temperature for up to two months. Once reconstituted, the solution should be used immediately, but if necessary, can be stored in a refrigerator between 2°C and 8°C for 24 hours.
Mechanism of Action:
Zoladex injection contains the active ingredient goserelin acetate, which is a synthetic analogue of gonadotropin-releasing hormone (GnRH). GnRH agonists stimulate the pituitary gland to produce and release luteinizing hormone (LH) and follicle-stimulating hormone (FSH), which in turn stimulate the production of estrogen and testosterone. Continuous administration of GnRH agonists like goserelin causes downregulation of the GnRH receptor, leading to a reduction in the production of these hormones. This can help to suppress the growth and activity of hormone-sensitive cancers, such as prostate and breast cancer.
Contraindications:
Zoladex injection is contraindicated in patients who are allergic to goserelin acetate or any other ingredients in the injection. It is also not recommended for use in pregnant or breastfeeding women, as it can cause fetal harm. Zoladex is not indicated in pediatric patients.
Interactions:
Zoladex can interact adversely with other medications, including tamoxifen, warfarin, and certain herbal supplements. Patients should inform their doctor of all medications they are taking before starting treatment with Zoladex.
Side Effects:
The most commonly reported side effects of Zoladex include hot flashes, sweating, fatigue, headache, and decreased libido. Other possible side effects include changes in mood, muscle weakness, bone pain, breast tenderness, and bleeding or spotting in women. Rare but serious side effects can include osteoporosis and a higher risk of bone fractures. Patients should inform their doctor if they experience any unusual or severe side effects while taking Zoladex.
